Spring Break Camp 2018 Parent/Guardian Questionnaire
Dear Parents and Guardians,
We had the most wonderful time working with your campers during our 2018 Spring Break Camp this year. Our Savannah resident, Peter Roberts was impressed with your smart, funny, brilliant kids and we were too! Peter wanted to do something special for your kids when he returns for his show in May, as a token of thanks for his residency. He is going to create a "Headcase" piece of art based on each of your children that he intends to give to you the night of his show, with hopes that you and your children can attend. This event will take place at SixTwelve that evening from 6-9pm and then we'll move his art to the SixTwelve Gallery at Tower Theatre the next day, where it will be hanging for two months.
Peter asked us to get some information from you that would aid him in creating each work based on your child's interests and likes, so if you could please fill out this form for each of your children by April 14th, that will give him enough time to complete this project. If you have any questions, feel free to email me at firstname.lastname@example.org and I'll get back to you as soon as possible. We hope that you can all make it to the opening of Peter Roberts' "HEADCASES (OK!)" at SixTwelve during First Friday on Paseo, May 4th, 6-9pm! Tell your kids that we'd love for them to wear one of their "Oklahoma Headcase" headbands if they will, but please keep the art gift a secret. We want it to be a surprise!
